Services We Provide:

1. Use of Universal Precautions at all times
2. Patient Care: Assistance with personal hygiene, fall prevention, assisting with transfers, positioning, ROM, bathing and toileting (ADL’S).
3. Transportation: Drive client to or from doctors appointments or when client needs to go somewhere (Caregiver will be reimbursed for mileage if driving own personal vehicle).
4. Run Errands: Run errands for the client as needed.
5. Food Preparation: Assistance with meal preparation, assistance with feeding (if needed).
6. Light House Cleaning: Client’s room and surrounding area ONLY
7. Emotional Support: Companionship, general supervision and
   encouragement of physical activity.
8. Health Monitoring: Remind and assist clients with medications, follow the plan of care from Hospice, PT, OT, ST, or physician’s plan of care. Keep a record of services provided.
9. Caregiver Expectations: Use the proper protocols when using
   medical equipment, and good use of body mechanics.
10. Caregivers shall observe physical and mental state of client on a regular basis. (Report any changes to the family and to Capitola Home Care LLC. for further intervention).
11. Keep client clean, safe, and comfortable at all times.
